What is Melasma?

Melasma is a chronic pigmentation condition that causes flat brown or grey-brown patches, usually symmetrical, on the face. It occurs when melanocytes, the pigment-producing cells in the skin, produce excess melanin, most often on the cheeks, forehead, nose bridge and upper lip.

Melasma is not contagious, not harmful and does not turn into skin cancer. It is a relapsing condition. It may be visibly lightened and managed over time, but it is not considered permanently curable, and responses differ between individuals.

Causes & Risks

What Causes Melasma?

  • Sun exposure. Ultraviolet and visible light stimulate melanocytes. Malaysia’s ultraviolet index is frequently high, so unprotected exposure may darken patches.
  • Hormonal changes. Shifts in oestrogen and progesterone are recognised triggers, including pregnancy, oral contraceptives and hormone therapy.
  • Genetics. A family history of facial pigmentation increases the likelihood.
  • Heat and irritation. Tropical heat, harsh skincare, unsuitable laser settings, thyroid disorders and photosensitising medicines may aggravate pigmentation.

Risk Factors

  • Being female, although men may also develop melasma.
  • Fitzpatrick skin types III to V, common among Malaysians.
  • Pregnancy, hormonal contraception or hormone therapy.
  • A family history of melasma.
  • Regular sun exposure without adequate protection.

Having a risk factor does not mean melasma will develop. Suitability for any treatment is determined following an individual medical assessment.

Signs and Symptoms

aMelasma is a visual concern and does not usually cause itching or pain. Reported signs:

  • Flat brown or grey-brown patches.
  • A symmetrical pattern across both sides of the face.
  • Patches on the cheeks, forehead, nose bridge, upper lip and jawline.
  • Pigmentation that darkens with sun or heat.

How Melasma is Diagnosed

Diagnosis begins with a face-to-face consultation. Your doctor will usually:

  • Review your medical, hormonal and skincare history.
  • Examine the pattern, symmetry and distribution of the patches.
  • Use a Wood’s lamp where appropriate, to assess pigment depth: epidermal, dermal or mixed.
  • Discuss options that may be suitable.
  • Pigment depth influences the approach taken, which is why diagnosis precedes any procedure.

Treatment Options

No treatment is suitable for everyone. Following an individual assessment, your doctor may discuss a combination approach, which could include:

  • Pico Laser: a picosecond laser that may be used to target pigment, at settings selected according to skin type.

  • Signature Peel: a chemical peel that may assist with surface pigment.

  • Microneedling Rejuvenation: may support skin renewal.

  • Skin Booster and Exosomes: may support skin barrier health and tone.

  • Topical or oral therapy, prescribed only where clinically indicated.

  • Discuss hormonal medication with your doctor.

If Melasma is Left Untreated

Melasma is not a dangerous condition. Without management, however:

  • Patches may darken and spread with continued sun exposure.
  • Deeper dermal pigment may be more difficult to treat later.
  • Unsupervised treatment may lead to rebound pigmentation.

Prognosis and Living With Melasma

Any improvement is gradual. With appropriate treatment and consistent sun protection, melasma may be visibly lightened and kept stable, although recurrence remains possible. Can melasma be cured permanently?

Melasma is generally managed rather than permanently cured. With suitable treatment and consistent sun protection, it can be visibly lightened and kept under control, but it may recur with sun exposure or hormonal changes. A personalised maintenance plan helps sustain results.

How many sessions will I need?
