What is a pico laser?

Pico laser is a non-ablative laser treatment that delivers energy in picoseconds, trillionths of a second. Because each pulse is so short, the laser works mainly through a photoacoustic (vibration) effect rather than heat. This is designed to break pigment into very fine particles that your body can typically clear naturally, while aiming to limit heat damage to the surrounding skin.

How pico laser works on the skin

  • Short laser pulses are used to target selected pigment concerns. The body may gradually process the treated pigment over time, depending on individual skin condition and response to treatment.
  • Lower heat means a reduced risk of post-inflammatory hyperpigmentation (PIH), an important consideration for Asian skin.
  • Certain settings also stimulate the skin’s natural collagen response, which can help soften the look of scars, pores and fine lines.

In simple terms, Pico laser may help target pigmentation and support the skin’s natural renewal process. Improvements can develop gradually, and outcomes may vary depending on individual skin condition and treatment suitability.

What Skin Concerns Can Pico Laser Treat?

Pico laser is versatile and may be used to address a range of common skin concerns. Suitability is always confirmed at consultation.

Pigmentation and melasma for Malaysian skin

Under Malaysia’s strong sun, pigmentation concerns such as sun spots, age spots, freckles and melasma are extremely common. Pico lasers can help fade many of these by targeting pigment at different depths.

An honest word on melasma: melasma is a chronic, easily-provoked condition. Used incorrectly, lasers can sometimes make it worse rather than better. At A.M Clinic, melasma is approached cautiously, often with conservative settings, supportive skincare and realistic expectations and it typically requires ongoing management rather than a one-off “cure.” We will be candid with you about what is realistic for your skin.

Acne scars and skin texture

By prompting collagen remodelling, pico lasers can help soften the appearance of certain acne scars, enlarged pores and uneven texture over a course of sessions.

Dullness, tone and rejuvenation

Many patients use lighter pico settings as part of a maintenance routine to support a brighter, fresher-looking complexion and a more even tone.

Tattoo removal

Pico laser is also used to break down tattoo ink so the body can clear it gradually. The number of sessions depends on ink colour, density and age of the tattoo.

Benefits of pico laser

Pico laser technology offers distinct functional characteristics, though individual experience varies:

  • Designed to target selected pigmentation concerns, with treatment settings adjusted based on individual skin condition and suitability.
  • Lower risk of post-inflammatory hyperpigmentation, which matters for Fitzpatrick III–V (common in Malaysia).
  • Downtime is generally limited, with temporary redness that may settle within hours to a few days.
  • May require varying session frequencies depending on the condition being treated.
  • Can be tailored to different concerns and depths within one platform.

Pico laser facial and pico toning

“Pico facial” (sometimes called pico toning) usually refers to a gentler, lower-energy treatment aimed at overall brightening, refining tone and supporting a healthy glow, rather than aggressively targeting a single deep concern. It is popular as a periodic maintenance treatment. Targeted pigmentation or scar work, by contrast, uses different settings and is planned around your specific concern. At consultation we will recommend which approach suits your goals.

Not sure if pico laser is right for your skin?

Book a consultation and have your concern assessed by a certified practitioner before you commit to anything.

Is pico laser right for you?

Pico laser suits many people, but it is not for everyone, and a face-to-face assessment is essential. You may be a suitable candidate if you have pigmentation, mild-to-moderate acne scarring, uneven tone or an unwanted tattoo and you have realistic expectations about gradual improvement.

When pico laser may not be appropriate

As a general guide, the treatment may need to be deferred or avoided if you:

  • are pregnant or breastfeeding;
  • have an active skin infection, open wound or inflammatory flare in the treatment area;
  • have a history of keloid (raised) scarring;
  • are taking photosensitising medication (for example isotretinoin/Accutane) or have
  • recently completed it;
  • have very recent sun exposure or sunburn.

Risks, Side Effects and Possible Complications

Pico laser is generally well tolerated when appropriate treatment settings are selected following a clinical assessment. However, all laser treatments carry potential risks, and individual reactions can vary depending on skin type, the condition being treated, treatment intensity and adherence to aftercare instructions.

Common and temporary side effects

Temporary effects following treatment may include:

  • redness or warmth in the treated area;
  • mild swelling or tenderness;
  • itching, dryness or skin sensitivity;
  • temporary darkening of treated pigmentation;
  • mild flaking, pinpoint crusting or a sandpaper-like texture.

These effects usually improve as the skin recovers, although the duration may vary according to the treatment settings and area treated. Clinical research on fractional picosecond treatments has reported temporary redness, swelling and itching among the more commonly observed reactions.

Less common risks and complications

Less common complications may include:

  • post-inflammatory hyperpigmentation, where the treated area becomes darker;
  • hypopigmentation, where the skin becomes lighter;
  • prolonged redness or swelling;
  • blistering, burns or infection;
  • worsening or recurrence of melasma;
  • temporary changes in skin texture;
  • scarring, although this is uncommon.

Pigment changes may be of particular concern in darker skin tones and after sun exposure. Appropriate patient selection, conservative treatment settings, sun protection and careful aftercare may help reduce this risk, but they cannot eliminate it completely.

For tattoo removal, certain ink colours may respond unpredictably, and some cosmetic or light-coloured pigments may darken after laser exposure. Complete tattoo clearance cannot be guaranteed.

Contact the clinic promptly if you experience increasing pain, marked swelling, extensive blistering, discharge, signs of infection or a skin reaction that is worsening rather than improving. Your practitioner will explain the risks relevant to your skin and proposed treatment during consultation.

What to expect: your treatment journey

  • 1. Consultation and skin assessment

    Your skin is examined, your concerns and history discussed, and a tailored plan proposed. This is also when realistic outcomes, session counts and costs are explained — with no pressure to proceed on the day.

  • 2. The procedure

    The treated area is cleansed, and numbing cream may be applied for comfort. Some patients may feel a light tingling or snapping sensation during treatment. Session duration varies depending on the treatment area and personalised plan.

  • 3. Aftercare and recovery

    Mild redness or slight swelling is normal and usually settles within hours to a few days. In Malaysia’s climate, diligent sun protection (a broad-spectrum SPF, reapplied) and good hydration are essential to protect results and reduce the risk of pigmentation rebound. Avoid harsh activities and exfoliants until your skin has recovered, and follow the specific aftercare your practitioner gives you.

Pico Laser Downtime and Recovery Timeline

Pico laser is generally associated with limited downtime, but recovery is not identical for every patient. The expected timeline depends on the treatment purpose, treatment area, laser settings, skin type and individual response.

Immediately after treatment

The skin may appear red, warm or mildly swollen. Some patients describe a sensation similar to mild sunburn. Treated pigmentation may temporarily look darker, while tattoo removal areas may develop more noticeable swelling, whitening, pinpoint bleeding or crusting.

The first 24 to 72 hours

Redness and swelling may begin to settle during this period. Patients receiving gentler pico toning or pigmentation treatments may feel comfortable returning to desk-based work or routine activities on the same day or the following day.

More intensive fractional, scar or tattoo treatments may produce visible redness, swelling or crusting for several days. One clinical study of fractional picosecond treatment reported that redness and swelling lasted an average of approximately four days, illustrating why recovery time can vary between treatment protocols.

Over the following several days

Temporary darkening, dryness or light flaking may occur as the treated area recovers. Do not scratch, pick or remove any crusting, as this may increase the risk of infection, pigment changes or scarring.

Use gentle skincare and broad-spectrum sun protection as directed. Strong exfoliants, retinoids, acids and other potentially irritating products should only be restarted when advised by your practitioner.

Returning to work and normal activities

Many patients do not require formal leave from work, particularly after lower-intensity treatments. However, you may prefer to allow additional recovery time if your treatment involves stronger settings, a large area, acne scars or tattoo removal.

Your practitioner may advise temporarily avoiding strenuous exercise, excessive heat, saunas, swimming and prolonged sun exposure. Normal activities should be resumed according to how your skin is healing and the personalised aftercare instructions provided by the clinic.

Downtime estimates are provided as general guidance only. Your practitioner will explain the expected recovery period for your specific treatment plan before proceeding.

Realistic results and number of sessions

Results develop gradually. Most pigmentation, uneven skin tone and acne scar concerns may require around 3 to 6 sessions, usually spaced 4 to 6 weeks apart. Tattoo removal may require 6 to 10 sessions or more, depending on the ink and tattoo characteristics. Results vary, and your doctor will recommend a suitable treatment plan after assessment.

Frequently Asked Questions

What is pico laser and how does it work?

Pico laser is a non-ablative laser that delivers energy in very short pulses. It may be used to target selected pigmentation concerns by breaking down pigment particles, which are then gradually processed by the body. Depending on the treatment settings and individual suitability, it may also support the skin’s natural renewal response.

Is pico laser safe for Malaysian or darker skin?

When performed by a trained practitioner with appropriate settings, pico laser is considered a commonly chosen laser option for Asian and darker skin (Fitzpatrick III–V), because lower heat means a reduced risk of post-inflammatory hyperpigmentation. Suitability is always confirmed at consultation.

How much does pico laser cost in Penang and Malaysia?

Pico laser treatment fees may vary depending on the treatment area, skin concern, number of sessions recommended, and individual assessment. A.M Clinic will advise the applicable fee during consultation before treatment is carried out.

How many sessions will I need and when will I see results?

It varies by concern and skin response. Many concerns need a course of several sessions spaced a few weeks apart, with results building over weeks rather than overnight. Your practitioner will give you a realistic plan; we don’t promise specific outcomes.

Does pico laser hurt, and how much downtime is there?

Treatment sensations may include mild tingling or a rubber-band-like snap, depending on individual sensitivity. Numbing cream may be used for comfort, and temporary redness may occur after treatment.

Can pico lasers treat melasma?

It can help, but melasma is a chronic, easily-provoked condition and laser used incorrectly can worsen it. We approach melasma cautiously, often with conservative settings, supportive skincare, strict sun protection and ongoing management — not as a one-off cure.

How is pico laser different from older or Q-switched lasers?

Traditional Q-switched lasers operate in nanoseconds and rely heavily on heat. Pico lasers operate in picoseconds, producing a photoacoustic (vibration) effect. This difference in mechanism is designed to fragment pigment finely while aiming to manage heat exposure to surrounding tissues. The right choice still depends on your skin and concern.

Who should avoid pico lasers?

As a general guide, it may be deferred or avoided during pregnancy or breastfeeding, with active skin infections or open wounds, a history of keloid scarring, while taking photosensitising medication such as isotretinoin, or after recent sunburn. Your full history is reviewed at consultation.

What is a pico facial or pico toning?

A gentler, lower-energy treatment focused on overall brightening and tone rather than aggressively targeting one deep concern. It’s popular as a periodic maintenance treatment. We’ll advise whether a pico facial or a more targeted plan suits your goals.

How should I prepare and care for my skin afterwards?

Avoid recent sun exposure and harsh active ingredients before your Pico Laser treatment, and let us know about your medication and skincare routine. Afterwards, protect your skin with broad-spectrum SPF, keep it well hydrated, avoid strong exfoliants until recovered, and follow the aftercare advice provided by your practitioner.
